Your First Visit to the Retina Center of South Florida

We are devoted exclusively to the medical and surgical treatment of the eye - specifically the retina, vitreous, and macula.

At our Delray Beach office, we strive to preserve the highest standards of quality medical care in a comfortable, caring environment. Here is what you can expect during your first visit with us.

Once you have made your initial appointment, you may pre-register by downloading and printing the forms below. Please fill them out and bring them with you to your first visit. Completing the forms prior to your visit will help speed up the new patient registration process.

Downloadable Forms:

  • New Patient Registration Packet

Please note: It may take a few minutes to load the forms if you are using a slow internet connection. If you are unable to view the forms, you may need to download Adobe Acrobat for free by clicking on the icon below.

What to Bring

Please bring your glasses, a list of your medications, your insurance cards, a photo ID, and the completed registration forms.

What to Expect

At your first visit, a medical technician will take your medical history, check your vision, measure your eye pressure, and dilate both eyes. The doctor will review your history and examine your eyes. He may advise additional tests.

Your first visit will be approximately two hours. Please make arrangements for a ride home, as both of your eyes will likely be dilated.

Should you have any questions about your first visit, our friendly staff is happy to help. Call us in Palm Beach County at (561) 499-8830.
